To work with us, simply fax your submissions to us at (800) 505-1058. You may use a standard ACORD form or one of our NFIB request for quote forms found in our agent's kit. Be sure all submissions include a fax cover page with your agency contact information, in case our underwriters have any questions or requests for additional information. If you have not already completed an agent agreement, we will ask you to do so when you bind your first policy. 

We recommend that you download a complete agent's kit, which contains all the necessary information and tools needed to have success with our program. The agent's kit contains general information, commissions, eligibility lists, state availability, request for quote forms and the agency agreement.

If at any time you have an underwriting or eligibility question, please contact our Business Insurance Center at (800) 505-6342 and ask to speak with an underwriter. You may also e-mail your questions or submissions to our staff at Business.InsuranceCenter@nfib.org. Our hours are Monday through Friday from 8 a.m.–5 p.m. Central time.
