ATTENTION: The Board has recently adopted revised C-2 and C-3 forms. All parties are to begin using these new forms no later than January 1, 2009.
To help in closing out the Special Disability Fund the Board has sent a letter to each self-insured employer, all compensation insurance carriers and the State Insurance Fund . The Board has also prepared answers to common questions to assist in this process.
