For dates of service from Oct. 1, 2008, through
Sept. 30, 2009, the
conversion factors are as follows:
- for medical/surgical services in part 5221.4030: $80.74;
- for pathology/laboratory services in part 5221.4040:
$67.43;
- for physical medicine/rehabilitation services in part
5221.4050: $70; and
- for chiropractic services in part 5221.4060: $58.14.
